Per MDV Lidocaine HCl. Per Polyamp inj Lignocaine HCl
Indications/Uses
MDV: Major nerve blocks eg, sciatic & retrobulbar. Polyamp: Local anaesth by epidural block.
Dosage/Direction for Use
MDV Adult & childn >12 yr Retrobulbar block 80 mg (4 mL) in 3-5 min for 1.5-2 hr. Sciatic block 300-400 mg (15-20 mL) in 15-30 min for 2-3 hr. Max: 400 mg (20 mL). Polyamp Adult 70 kg Epidural anaesth (lumbar) Individualised dosage. 100-200 mg (5-10 mL). Max: 3 mg/kg.
Contraindications
Hypersensitivity to amide-type local anaesth. MDV: Hypersensitivity to methyl- &/or propyl parahydroxybensoate or PABA metabolite. Avoid use in patients allergic to ester local anaesth. Polyamp: Uncorrected hypotension or coagulation disorders; inflammation &/or sepsis in inj region, presence of septiceamia. Concomitant use w/ anticoagulant therapy.
Special Precautions
Not to be used for post-op IA continuous infusion. Porphyric patients. Inadvertent intravascular; retrobular & peribulbar inj. Insert IV cannula prior to local anaesth inj. Concomitant use w/ class III anti-arrhythmics eg, amiodarone. May affect ability to drive & use machines. Severe hepatic disease or renal impairment. Pregnancy & lactation. Elderly, young or debilitated patients. MDV: Not to be inj intrathecal, intracisternal & intrabulbar routes. 2nd- or 3rd-degree AV block. Polyamp: Known drug sensitivities. Epilepsy, impaired cardiac conduction, bradycardia, severe shock or digitalis intoxication, impaired CV function, Stokes-Adams or Wolff-Parkinson-White syndrome; hypovolaemia; pre-existing abnormal neurological pathology eg, myasthenia gravis; CNS or spinal cord diseases eg, meningitis, spinal fluid block, cranial or spinal haemorrhage, tumours, poliomyelitis, syphilis, tuberculosis or spinal cord metastatic lesions. Monitor CV & resp vital signs after inj; signs & symptoms of neurological impairment; fetal heart rate. Avoid subarachnoid inj. Indwelling epidural catheters, traumatic or repeated epidural/spinal puncture. Concomitant use of drugs affecting haemostasis eg, NSAID, platelet inhibitors or other anticoagulants.
Adverse Reactions
Hypotension. Bradycardia; dizziness; vomiting. MDV: Nausea. HTN; paraesthesia. Polyamp: Lightheadedness, nervousness, apprehension, euphoria, confusion, drowsiness, tinnitus, hyperacusis, blurred vision; heat, cold or numbness sensations; twitching, tremors, convulsions, unconsciousness, resp depression &/or arrest, agitation, difficulty swallowing, circumoral paraesthesia, tongue numbness, slurred speech; CV collapse; maternal hypotension.
Drug Interactions
Potentiated cardiac effects w/ other local anaesth or structurally-related agents to amide-type local anaesth eg, certain anti-arrhythmics eg, disopyramide, procainamide, mexilitene. Reduced clearance of amiodarone. Reduced metabolism w/ β-blockers eg, propranolol, metoprolol. Reduced clearance by cimetidine. Enhanced metabolism w/ phenytoin, other antiepileptics eg, phenobarb, primidone, carbamazepine. Additive cardiac depressant effects w/ phenytoin. Decreased min effective conc of inhalational anaesth eg, nitrous oxide. Excessive neuromuscular blockade w/ skeletal muscle relaxants eg, suxamethonium. Alkaline soln. Interference w/ creatinine & creatine kinase levels. MDV: Additive toxic effects w/ other local anaesth or structurally-related agents to amide-type local anaesth eg, certain anti-arrhythmics (eg, mexilitine, tocainide), class IB antiarrhythmics. Potential toxic plasma conc w/ cimetidine.
